We are committed to meeting and exceeding the standards set out in Alberta’s Continuing Care Health Service Standards (CCHSS). Long term care is committed to caring for individuals with complex, unpredictable medical needs who require 24-hour onsite Registered Nurse assessment and/or treatment. We provide safe, quality care, and emphasize culturally appropriate practices to enhance the quality of life for our residents. 

We strive to maintain a home-like environment for our residents. Our team of professionals include:

  • Pharmacy Services
  • Palliative and End of Life Care

We incorporate western and traditional practices to address the resident’s needs and to enrich the health of the residents.

For on-reserve Continuing Care Services, we recommend that Community members follow this process:

Step 1: Contact your local Home Care program in your area.

Step 2: Home Care completes assessment and is sent to the Continuing Care Access Centre which will provide assessment and refer clients to Continuing Care services and program within their community.

Step 3: Continuing Care Access Centre reviews assessment

Step 4: Resident is contacted.

Referrals for Continuing Care can be submitted by:

  • Family Members
  • Physicians
  • Other Long Term Care Facilities
  • Hospitals
