Indigenous Patient Navigator
FTE: Full-time permanent

Reporting To: Director of Non-Insured Health Benefits
Summary of Work:
Under the supervision of the Director, the Indigenous Patient Navigator’s primary responsibility is to support patient’s unique health goals, by guiding them to appropriate medical professionals and reducing potential barriers and access to medical care. The Navigator is responsible to build connections and collaboration with a multitude of health professions in and surrounding areas of the Blood Tribe.
Personal Qualifications:
- Knowledge of the health care resources in and around southern Alberta, including Blood Tribe services and entities.
- Knowledge of surrounding services, Alberta Health and Alberta Health Services.
- Knowledge of the Blackfoot culture and language
- Strong interpersonal communication, teamwork.
- Ability to maintain a high level of confidentiality.
- Strong awareness of appropriate professional and personal boundaries.
- Excellent oral and written communication skills
- Knowledge of computer programs, i.e. Word, Excel, Outlook.
Professional Qualities:
- Diploma or Degree in Child and Youth Care, Social Work, Nursing, Human Services Degree, allied health professional designations/health science or equivalent.
- Valid Drivers License
- Minimum 3-5 years of experience providing Indigenous patient and cultural support.
- First Aide and CPR certification
Salary Range:
- In accordance with the Blood Tribe Department of Health Inc., salary grid.
